If you are a new patient, please arrive ten minutes prior to your scheduled appointment time to complete your registration and have your chart prepared. You will then be greeted by one of our ophthalmic technicians, who will escort you to the exam room, take your history information, and perform some preliminary testing on your eyes. Don’t forget to bring a list of your current medications and information regarding your general health and any ocular conditions. You will then see your doctor, who will carry out additional testing, explain the results of your examination, and discuss treatment options with you. Most children under the age of 14 will need to have their eyes dilated as part of a comprehensive eye examination. This procedure allows accurate measurement of the focusing ability (refractive error) of the eyes and proper examination of the back of the eye. The eye drops used in the dilation of children’s eyes are stronger than those used in adults. Consequently, the dilation and blur caused by the drops typically lasts up to 24 hours, making it difficult to read or play sports the rest of the day after the exam.

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.

If you do not have a copy of Acrobat Reader you can get one for free by clicking on the icon ‘Get Acrobat® Reader’. This will take you to Adobe’s website and enable you to download the Acrobat program to your computer.
Cancellation Policy
With the exception of serious emergencies, it is expected that you keep all of your appointments. If you need to reschedule an appointment we require 24 hours notice. In such a case, please call our office and arrange for a make-up appointment with one of our patient care coordinators at 805.495.0458.
Insurance & Financing
We are providers for Medicare, most PPO insurance plans, and most plans offered by VSP (Vision Service Plan). Please contact us if you have any questions regarding your insurance coverage.
All co-payments and deductibles are due at the time of service. This includes payment for any surgical procedures, which is usually taken care of in advance.
The Lynn Eye Medical Group accepts cash, personal checks, credit and debit cards. Lynn Eye also offers financing through CareCredit. Click on the card below to sign up for this service:
CareCredit helps you pay for out-of-pocket healthcare expenses for you and your family. Once you are approved, you can use it again to help manage health, wellness and beauty costs not covered by insurance. Please note this service subject to credit approval.
