(We are no longer open on Saturdays)

Our patients mainly live in Thousand Oaks, Newbury Park, Westlake, Moorpark, Agoura and surrounding areas. However, we are happy to provide eye care to patients that live in other areas. If you have any questions or comments, or if you want to learn more about the services we provide, please complete this online form. However, we are unable to answer specific medical questions or give individual medical advice using this form.

Surgeries are scheduled in one of the two Eye Surgery Centers:
LYNN Eye Surgery Center (located in the same building as the office at 2230 Lynn Road, Ste 106, Thousand Oaks, CA) and
WESTLAKE Eye Surgery Center, 2900 Townsgate Rd., Suite 201, Westlake Village, CA
